User Tick/Check List Module

I want a module which does the following:

1) Enables Admin to create a List of items and add/edit it.

2) Enable Users to Select the individual items and add a comment.

The item I want it for is Birds, so users can have a list of sightings.

Does anyone know of a mod which can do this?:confused:
