Um.... copy the sendmessage.php page and the templates used?

Not really any good as it would use the subjects from the standard 'Contact us' form

indicate the goals, so the suggestions will be better.

Subject line?
the settings in the standard contact form inserts the subjects available

OK, if you don't want to start with sendmessage.php, then start with one of these - How to create your own vBulletin-powered page! (uses vB templates) or [How-To] vBulletin API Basics: Creating Custom Pages & Misc. - then google "php forms" and you can write your own without the standard subject (although why you wouldn't want to just remove that part from sendmessage.php instead, I don't know).

If you want a very easy solution then try http://jotform.com/

We use it on some of our sites. However there are plenty of good form scripts available in php etc.
