problem with notifications

my webhost recently upgrade to php 5, I am not sure theis may be the problem but now my drop down list for notifications are not working. Can anyone help me with this issue?

well i found that it culd be something done with the template because it works in all of the other templates.

Create a new style with no parent set. Does the issue still affect this style?
