How to replace favicon with custom one

I screwed something up and now it looks goofy (at least it does in FF).

How do I start over and place a favicon so it will display?

I cleared my cache and it now is visible in Firefox but not in IE??
And it doesn't show on my HOME page which uses vBAdvanced. I even put the code into the Portan template's head section.

I see a favicon on that page - but I'm on firefox.
