Need help creating a top 25 poll
 

So I want to make a poll that will basically have 25 drop down boxes numbered 1 through 25. Each drop down box will have every team possible. You pick the team and then the poll will calculate the overall standings and we can display the results on a separate page at a later date. It has to IP log and it also has to be able to check to make sure that someone doesn't put the same team in mutiple boxes. I believe I can find the way to create the mysql database for it, but I am having a hard time finding a tutorial on creating the php side of this for the poll gui. If anyone has any suggestions on possible tutorials or can help guide me through some of the stages it would be much appreciated.
