Any type of application mods?
 

At one of the sites I run, we develop various items. What we've been looking for is an add-on where it would add a link to the navbar, maybe something like "Apply" You would click that, and be taken to a form. This would have a drop-down menu where you could list various items like, "I want to be a mod" "I want to be a developer" "I have a suggestion for the site" etc
Then, based on what you chose, you would get a set of custom fields to enter the appropriate information. When you're done, it could send a pm to a specific user, or maybe make a topic on a board.

I saw some of the support ticket mods and such, but none of them really fit what I was looking for, any suggestions?

Take a search with keyword Moderator Application,

and then do some litte change with your navbar

and Templates on that Mod.
