Looking for a hack that create like simple "fields" inside a new thread
 

Hi,

I would like that when a user click on "new thread" inside the window where they write, there are already "fields" to fill (im not talking about such thing as "form hack"), then they just have to fill and click "ok" (i don't want this fr to post of replies of course)
Is there a hack that can do that?

i.e: i click on "new thread" the window appears (the same as im writing right now) with this kind of thing:

Your blabla:


Are you?

Describe blabla:
