Is there an add on for the Sub forums Icons as shown on this forums?

You can see the SUB FORUM then the small ICON.

Is this a custom set up?

You mean the red bullet/dot next to the sub-forum names? vbulletin options > Forum Listings Display Options > Depth of Sub-Forums set that to 1 (or something other than 0)
