Adsense placement + Style changes not saving

Ive got my code,

Id like to have the ads appear as per the attached image, how can I do this? Id also like these ads NOT to appear on the forum index..?

Also ive tried applying and saving this code to the header in styles, but the changes never save....doesnt matter where I put the code...why?
