Forms for posting threads
 

I want to do something similar to how you'd post a Mod in vbulletin.org forums - there are a few questions you need to answer apart from your post.
Based on those answers, a neat table is drawn out at the top of the first post - so people can see the details.

How does vbulletin.org do this :confused: ?
