Registration field sent as an email?

Is there anyway I can place an optional field in the registration page that will send me an email for what was typed in that field.

Basically I'm going to try and track a number.

User gets a card asking to register and enter the number on their card as well as their zip code.

They then pass the card onto someone else who does the same.

And eventually using the info i can map out where the card has been.

basically i want something like this integrated into the registration page.
