Use of a single custom template

Hi all,

I've tried looking at some help pages for this, but they all throw me in a loop:

All I'm trying to do is connect my custom template, lets call it customtemp and refer to it within my forumhome as $customtemp . Could anybody refer me to the simplest way possible?
