Advanced Contry and State Mod

Hello,

I would like to make request for this mod.
There are several Country mods but they are not what I need

To see what i mean check :

http://www.masterross.net/country.jpg:

1. user need to "Select Country" in first drop-down menu (for example USA)

2. list of all USA states (regions) will be loaded in second drop-down menu and ask to select some.

(dont know how many mysql queries should be used here i'm not programmer)
