welcome / accept page

how do i make a page that loads when any page is loaded on the forum with rulez and an accept button and if accept is pressed it wont load again but if its declined it keeps loading until it is accepted also when its declined can it keep a record of the ip / username of who declined it.

urgently needed if someone wont make it at lease give me a tip where to start
