Form Option for Idiots
 

I love the poll feature and would want something similar for forms. I want to be able to have a infomation request form whereby users select various things they want to receive from us. For instance:

a button or check box for a Free DVD - and a text entry for name & address to mail it to

a button or check box for pdf files via email - with text entry for email address they'd ilke it sent to

a button or check box for a Sales Rep to call with a text entry for tel #, time zone and best time to call

I did download the program "forms" but don't know coding and was totally overwhelmed at what was required to do to customize it for us. The vBulleltin poll option is totally idiot-proof. That's what I'd like to see for forms.
