PLEASE! - Tell me how this is done!
 

All i need to figure out is how to do this! I am so frustrated right now.

I need to create a simple form that will tie into a custom product i am building.

But when I want to create a plugin, I need to know a hook to use.

I have a portal page that asks the user to ADD, they goto this page and add data into my custom table.

Now, i can handle the php inside the plugin, but how do i go from point A to point B.

Can anyone help or point me to a help file.
