displaying poll from vba on photopost page

hi, i dont know how to do this, i know its possible ive seen sites with this

can someone please tell me what is the entire code i need to place on my photopost page (i have a place to put it already so it will show on the left sidebar) for it to display a random poll from the forum, which i already created this poll on vba and its currently already showing on my vba page , but i want to also have it show on the photopost page, but i dont know how.
