Post templates / mod question?

What I am trying to do is...when a user creates a new thread for a certain section of my forum, I want them to be presented with a form (I'm thinking of using the "form hack" mod).

I want the data they enter to be layed out a specific way in the post. Does the "form hack" do that, or do I need to create some sort of "post / thread template"? I didn't know if there was anything already out there that does this.
