Form with Thread Fields --> Post

I dont know how to explain this, but I want to have a form. From that form, the fields will be entered into a new thread on a particular forum.

However, i want to be able to format the form fields... IE. Font size, color, bold, etc.

The closes I found was something like this https://vborg.vbsupport.ru/showthread.php?t=108888 however, I want it to be completely customized (the form) and the entire form to be IN THE POST, not in the thread title or whatever.
