Pagination loses query data on 2nd page

Hello,

I've a complex query based on data from a form. The query executes fine when I don't have pagination but when I'm adding pagination then query losts the data after 1st page. See the code below. Also could you please someone advice me how I can mix the 2 queries in 1? I mean "Select records" and "Count records" in the same query as the "WHERE" is the same.
