New Thread Requirements mod / Format user friendly
 
How to format new threads modification needed?!?
Ok I hope I explain this good.

I have a forum where I want things to be displayed a certain way. So let's say I have a big forum where a bunch of music gets post. Is there anyway under one forum that I can have it display the following when someone posts a new thread:

song artist: [text field]
song title: [text field]
song link: [text field]
-POST-

And then when this information is posted it will post the album as a new thread like this as the title:

[song artist] - [song title]
And have the content just the link

Make sense? Anyone guide me to the right direction?
