How would I create a forum from outside of my forum directory?

I'm mainly wanting to know what SQL queries need to be made to add a forum from a page outside of my forum directory.

I have a form that lets users create their own forums and want to make sure I do all the necessary SQL queries to get a forum setup correctly.

Anybody?
