Need a Modification to Contact Us Page willing to pay.
 

I am willing to pay someone to do this modification for me.

What I need done:

I need a questionaire style page to be placed infront of the contact us page, so when a user clicks contact us, this page appears, and then a link at the bottom of said page forwards them to the actual contact us page after they have read the new page.

On the new page I need the following (or along those lines).

Are you contacting us Regarding Your account?

Forgot your Password? Reset your Password HERE

Forgot your Username? Fill in this Form

Forgot your Username and Email address? Were Sorry, but nothing can be done to help you, you need to register a new user name.

This is very easy.
All you have to do is create a new custom template with all options/forms listed above.
Then use the sendmessage hook to call the new template, above your actual sendmessage form.

This is a good idea, I will add it to my sendmessage form to. :)

I dislike template mods and prefer products. Reason being: I like doing upgrades to vb with as little problems as possible.
