I was wondering if there is any hack like this:

1. You'll need to edit the default or custom Form plugin (set questions and answers there); set templates used there.
2. Edit "form" or custom template for questions.
3. Edit "formanswers" or custom template for answers.

It's quite a bit of work, however, to customize it fully. It took me a while. :)
