Mod release hack - like used here

Iam looking for the mod that you use here that when a member releases a mod they input the details of the mod then give a discription and the them upload the zip and screen shots

All information is discplay at top next to his name etc

Is this available and can we use it and change the wording off the input areas?? to use with other stuff apart from vbulletin mods
