You probably gave the forum "SMS Donation" broken HTML as forum title.

Looks ok, check the forum settings compare it to the two you have above see what is different

Since the button "Edit Forum" is gone, I guess you'll have to access the edit forum URL by using this

Edit forumid with the forumid of your broken forum category.

I'm not sure, you must of added something somewhere which is causing that.
Check your plugins, your forumdisplay templates, etc.
