i have installed this in the admin cp but it does not show on my forumhome :( any ideas ? is that all that was needed to do

There can be many causes - the most common ones being a broken hack somewhere stopping the hooks working, or you having a custom FORUMHOME template - but there is no real way to know without proper investigation - that requires admin access to your site.

installed, however I now have users who have visisted as well as members who have visited....

Any ideas as to why?

upgraded recently from 3.0.9 to 3.5.4

Any help would be greatly appreciated

You need to remove the old template code (from Forumhome) that the 3.0.9 version used.
