It is, but you need to change the template and the survey.php file. I've had this installed on my site since vB3.5 and have managed to keep it going all this time. I could try and port this to vB4 if there is enough demand for it. (And with the owners permission)

i was more of referring to the functionality of the modification. Many of the features do not work i.e. results, exporting, etc... not to mention styling issues

Its not often that I have to tweak the board i run, so i'm hardly an expert, so maybe I can get a little help with this Mod.

Problem Statement: When I navigate to Http://URL/forum/survey.php I see the source code of the survey.php, instead of what i would expect (a GUI).

I believe i followed the instructions for installation.
Survey.php in the forum root.
Survey folder in the forum root.
Installed the xml file by browsing to it on my computer.

All of this done through FTP.
Any help I can get would be great.
