Perhaps you should include an uninstallation guide or file, too. It adds tables to the database and for those of us that like to uninstall entirely may find that a bit irritating having to manually drop the tables.

Rasp I have asked about this before, it will be a feature in a future version. Right now there is uninstall instructions but its manual.

That really looks like a server/apache misconfiguration error, I would contact your server admin (hosting company) about that issue.
