To answer my own question for the benefit of anyone who may run into the same issue, the problem was the plugin was set by default to send notifications as User ID #1, which does not exist on our forum. We changed the user ID to a valid user and everything started working as expected.

Argh. Uni hasn't been the best experience for me, but now that I'm out of school for the summer, hopefully I can pick this thing back up and see what I can do.

For all the people using, testing, and supporting this while I wasn't around to, thank you! :)
