I can't say what is wrong with your database. But you can try one more, or just use PHPmyADMIN to add the line directly

mysql error: Unknown column 'GODFATHERID' in 'where clause'

mysql error number: 1054

It means you should to register new user with nick "Godfather",
which userid="GODFATHERID". The same with user "INSURANCE".
