New Joe: Try chmoding your /tmp/ directory to 0777 and see if that fixes the issue. If not then let me know and I have 1 more way of trying.

I was playing with this but found I can't upload files with it. I was going to use it for a handful of zip files I have for download, but I continuosly recieve an error that reads:

You have that wrong on the extensions, notice how I have image/gif, image/jpeg etc. That basically is telling it the extension is that of an image with the extension of .gif or .jpeg. What you need to do is change those you have at the end to this:
