Read FAQ entry #4 (especially question # 4) again and do the necessary things

Step 2
If you still get the same error, redo step 1

Right I understand that is how it should work, but it is not happening on my board. Should I try re-installing and over writing the .xml file?

Reinstalling won't help.
Make sure your settings (userid) are correct. If it still does not work, feel free to change ERRTYPE_SILENT to ERRTYPE_STANDARD to see errors that might occur.
Also note that if you are moderating members, the PM will be sent after a new member has been accepted.

I am not moderating new members, just checking the email addys and require the activation, could it have a conflict with the automatic welcome thread hack?

also, if I turn off email confirmation, it works just fine.

It could, but it shouldn't. I suggest to disable all other products except Automatic Welcome PM and test again.
