I'm having a problem with the attachment insert button

Here it is, working in the default style
And here it is in Morbid Blue, not working.

How can I fix this?

i upload and install style but no images and css workibg on my site? i upload digitaalvb on root and import xml in style but none images and css working on my site. what can be issue

Please update this style to vB 4.2.1, because there's many this style mistakes because of 4.2.1 version update.

I notice that this link:
<img src="digitalvb/morbid/statusicon/category_forum_old-48.png" class="forumicon" id="forum_statusicon_2" alt="">

The status icon is not included.

Category_forum_old-48.png does not exist. This PNG is not in the 4_2.zip so that icon doesn't load.

This is on 4.2.2
