Yeah same here. I tried it on 4.2.0 pl4 and the latest 4.2.2 and it doesnt work. Both on blank forums.

Updated to 1.0.1
-Added Picture as Username everywhere there is a musername
-Added Compatible with any mod that has musername If you Enable Cache.
-Added to the Options / "Places to have Username Picture" you can pick everywhere or just PMs, Profile, Post and Members List.
-Fix some bugs
