I had to create a new folder in within my 'misc' folder, and upload all the contents individually. /forums/images/misc/usercp_icons

Also as a note, I followed the instructions - uploaded the .gif files and install the xml.png - and it didn't work. uninstalled the .png and installed the other .xml and it worked.

Then i read the intructions again and came to the conclusion i didn't upload the whole folder
Upload the "usercp_icons" folder into your current style images/misc dir.
