i have the same problem and ad the SQL DB is Vb_4pt_issue but not pt_issue
in the table is the report i done test1

was for me.
have you all running patch?

I use vb4.2.2 and the latest pt
the problem i have is

I'll take a look. It seems this happens on mod_rewrite only.
