ok i am a complete blonde and i have everything uploaded right except for the Upload the files to the directories listed below.
forum/

- staffapp.php

Where do i go to do this??

I put mine in my root folder http://webcenter.cc

So just upload to wherever your site is.

Has anyone tried it on vBulletin 4.0.x to see if it works on that?

How hard is it for the developer to say that if anything is planned for the future release or not?
