If creating a new style with no parent is not working, then you have bigger problems than just human verification. If you have ticket support, then I would suggest putting in a ticket.

creating style with no parent works,
we've now upgraded to 4.1.5
still no HV for registration, contact us or password change request as it should be according to settings
any help?

Wow, you've been living with this since June 23? Why didn't you do as Lynne suggested and open a help ticket?

Rather than wait forever and let spambots accumulate, I would have just done a clean install of vb long ago.

To avoid spambot registration we activated manual moderation of all new registrations then. By the way the problem got sorted, the solution was found here:
