I would like the custom search to be the first search option, so at the top of the drop down.. How would I do this please?

I've done everything as stated in the guide but i do not see "Make Money" and the GCS Panel and fill in your AdSense ID anywhere. Where is it located exactly?

Ignore the above, this info must be filled in Google Custom Search page. Thanks!
