Can you be a little more clear as to where?

For some reason this mod isn't working for me.

It used to work, I don't know why it doesn't work anymore. Have there been any reports of conflicting plugins or anything?

How can I troubleshoot the addon?

Basically, no PM is sent when moderation happens. No errors anywhere or anything, just no PM sent.

EDIT: nevermind, had to change the setting on age from 0 to 100.

seems not to work fully anymore for me? deleting a thread does not give an PM anymore, but moving does?

I tested latest version (4.0.6) on my vBulletin 4.1.8 board and Moderation Auto-PM still works without problems. If Moderation Auto-PM doesn't work, it may be a conflict with another plugin or a bad configuration. Please remember that Moderation Auto-PM never send a pm to yourself.

As above, working fine on 4.1.9.

I've found some conflicts between this mod and VaultWiki https://www.vaultwiki.org/
Especially with thread move.

As kastak says, this mod works fine, but if you have issues it maybe a conflict with another mod.

I'll dig into this further, but what happens is the re-direct after the thread move fails.
Internally it looks like the $threadinfo object is messed up.
