How do you make it appear at the "top" of the Blog post?

Just stick it in random spots until you find the place you want for it to go. Look for visual clues such as links and variable names to give you an idea of where in the template you're inserting it.

I went to:
Admincp>Styles & Templates>Style manager>Edit templates>Show Thread templates>SHOWTHREAD

In the SHOWTHREAD template, I could not Find the following:

I see only the part in red, but not the rest.

What do you think is the problem? Should I just stick your code under there?

that should be the spot, see how similar it is? its just been updated at some point, and the initial post was (nor will be, as this mod is unsupported) not updated to follow.

The guy hasn't been back on this site since Dec of last year and his own site has expired so, that's why "No reply yet??????????"

It's an unsupported mod anyway, had been from the start.
Tho, its really not complicated, all he did was use the addtoany site to create the bar, then just found where to drop it in the template.
You can put this anywhere...just experiment with putting it in different places in the template till you find where you want it to go.
