is there other way to customize it? like other staff application name? warez crew, graphics crew.. etc.

Your sites name will appear in it, not "U.S. Military Life" if thats what you mean

When member click on send Application getting this error.

Sorry. I'm unable to duplicate this error. Do you have a forum selected for the form to be posted in? Does it get posted there? Does the member see the thank you page after submitting, or does it just show that error?

Is anyone else getting this error?
