All of your forums must be contained within a category for this style to work (You have forums out on their own, which won't work because of the category spacing modification that I applied.)

My forumdisplay pages are centered on IE. I would normally chalk it up to IE being IE, but it's only on forumdisplay, which has me really confused.

EDIT: fixed. It was a problem in my navbar. Still no clue why it was only affecting forumdisplay, especially since it was an open tag in the navbar. Oh well.

I am not getting the columns for last post, threads, num of posts at the top level forum. Basically it is the row under the first row with User CP, FAQ, Member List, ect. that is missing.

It is correct at level two and three. Where would I go to correct this?

Any help is greatly appreciated.

Are you placing all of your forum groups within categories ? You can't have a forum as the top level of a group due to the template modifications I applied to the style, so that's likely going to be your problem. If you could link me to your forum, I could let you know for sure what the issue is.
