Well this is my first hack for vBulletin, I hope you like it.
What this hack does is have anyone submit a testimonial on your site.
The first image is for admin view and the second is for everyone else.

Hey pretty funky..
I was going to start a feedback section on my site, this might work out pretty well I'll give it a try!
Maybe a good enhancement in the future is a way for admins to add a testimonial from someone ELSE that might have submitted a comment about the site via e-mail? There's a few nice things that have been said about our sites via e-mails or PM's and I'd hate to leave them out!

Where do these testimonials show up? A separate page? On forumhome?

Might be a good idea to moderate testimonials :)

I was thinking of making another row named "show" then when someone adds a testimonial it'll insert the value "no" then I was going to make a category for usergroups with something like "Can Moderate Testimonials" "Yes" "No".
Then in the modcp and admincp I would have a section for all the entries where "show" equals "no".
The moderator or admin can either accept or deny the testimonial
How do I do the badword filter?

Moderation would be good. If you had moderation on, you would not need a bad word filter, cause you could check it before accepting.
