More options in User Profile Fields

Is there any way I can add more than one question in a user profile field? Check out my image if you don't understand my intentions. Thanks.

I don't know how that first field was created, but i guess you can do this with User Profile FIelds.

ugh.. the first field was created in the Add User Profile Field menu... Can anyone please help?

Then just add more fields there.

I would, but I want to keep all of these under the same heading and beside eachother if possible.. If I a field for each it will not look good.. Is there a way to edit the User Profile Field code myself?
