Supported - Denotes whether the modification will be supported by the author
DB Changes - Denotes whether the modification will make changes to the database
Installer included - Denotes whether the modification includes an automated installer which may run the queries for you, and/or install phrases and/or templates
Uses Plugins - Denotes whether the modification will use the Plugin system in 3.5.0 or not
Template changes - Denotes whether the modification will require you to alter or add templates
Code changes - Denotes whether the modification will require you to alter existing vBulletin source code
Additional files - Denotes whether the modification requires you to upload additional files to your folder structure
Is in Beta stage - Denotes whether the modification is considered stable by the author - If this is checked, the modification is considered a beta release

ala-cart options - they are available so you the 'person downloading addon' have the option to choose how you would like your addon ;)

LOL, sorry, those options are actually for the author of addon and are there to show you some information regarding the addon -- I'm not sure why the checkboxes are available for EVERYONE--bad usability.
