How do I add the header/nav bar in a non-vB page?

Searched the forum and was unable to find the answer:

While I dig into the new "php for beginners" book I bought, could someone jumpstart me on how I would include the vB header and nav bar on a non-vB page?

I know I need to change directory, get the globals and eval, but don't know the proper syntax...
