Placing ads on the 2nd to last post

I want to monetize my forums as best as possible. I have Google Adsense at the top but the clickthrough is very poor.

Questions:

1. How can I place an advert between the last post and 2nd to last post in a thread? I've heard that gets a good clickthrough.

2. Anybody here with experience on how best to optimise returns from a forum?
