Looking for a working hack for 2.28 that will prevent users from uploading an avatar to my site and then linking to it for another site. it is really annoying.

If you have the appache modual mod_rewrite installed then use that to block domians other than yours to use images from your server.

will this code block access to avatars as well? do i need to install the save avatar as file hack? does that hack work with 2.28?

That should block all access from outside your domain's that you list instead of vbforums.com So all attachments and avatars :D
